You may download the form to your computer or you may complete the form online, however, please print a copy of the completed form and press submit to have the completed application form emailed to the IADDA offices. Once we receive your application, we will contact you to discuss your dues amount. Dues are based on the total amount of substance abuse funds you receive (including government and third party revenues).

Once your application has been approved, your user account for the member’s only section will be activated.
