Job Description
Maine Community Youth Foundation (MCYAF), a local community coalition working to reduce drug and alcohol use rates among youth, is in need of one part-time person, 20 hours per week. to coordinate a Social Norms Marketing Campaign and carry out administrative functions. Duties include: work with team to create marketing materials including posters, brochures, ads, and press releases; input data for analysis, project coordination, and Internet-based research. Familiarity with prevention strategies and a basic understanding of Social Norms Marketing a plus. this is an hourly, contracted position with no health benefits.
Requirements
A minimum of bachelor's degree in community/public health, social services, education, or related field; project leadership skills; creativity, design and marketing skills. Strong computer skills, including familiarity with Microsoft Office Programs and Internet research; excellent oral and written communication skills; ability to work comfortably with a wide range of people, including youth, parents, school administration and community leaders. Access to a car and ability to lift and transport program materials.

JOB SUMMARY
Basic responsibilities include the provision of direct casework assessment for Community Health Services; maintains contacts with the primary care providers to assure that the clients and his/her family receives appropriate health care services.
JOB DUTIES
-Responsible for providing direct case management services at any given time during the fiscal year
-Maintains ongoing contact with a minimum of 80% of assigned clients at all times
-Responsible for conducting client assessment of assigned caseload
-Responsible for developing service plans for assigned caseload
-Responsible for evaluating services provided to clients on assigned caseload and ensuring that services are adequate
-Ensures that services are implemented and coordinated for assigned cases
-Responsible for initiating appropriate linkages and referrals to all assigned cases
-Collaborates with PCP staff in providing comprehensive services to assigned caseload
-Collaborates with supervisor regarding program objectives as they relate to services
-Prepares reports for the Director and/or administrative staff regarding assigned caseload
-May attend executive meetings on behalf of the organization
-Data entry into computer system all aspects of assigned caseload
-Enhances professional growth and development through participation in education programs, workshops etc
-Other related duties as assigned
QUALIFICATIONS
-Bachelor’s degree; preferably in behavioral science or social science and at least 1 year experience in child, family, or community services
-Valid Driver’s License and vehicle required to travel to different programs
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
• Ability to analyze and solve problems
• Strong interpersonal skills, flexibility and client service orientation
• Ability to plan, organize and direct multiple clients and activities
• Maintains good working relations with Management and Administrative staff
• Ability to gather data, compile information and prepare reports
• Ability to manage case load

DESCRIPTION OF DUTIES
This position focuses on substance abuse prevention throughout Lake County. The program goals are to reduce substance abuse among youth and to strengthen the community awareness of and involvement with substance abuse prevention issues. Primary responsibilities for this program include: facilitating a Life Skills program for elementary/middle school youth in targeted areas; assisting organizations and municipalities in adopting or strengthening policies
related to substance abuse prevention; and community organizing by helping to develop effective substance abuse prevention focused coalitions. Peer education and expansion of a Social Norms Campaign are two other activities being implemented by this program. This position requires the
ability to work with a variety of populations including community stakeholders, parents, law
enforcement, school administrators, and youth. This position also includes maintaining appropriate documentation, demonstrating accountability for work through timely submission of
data, and assists in development of annual program goals, and in writing required reports. Local travel required within Lake County. Will participate in staff meetings and assigned training activities and actively participates in program evaluation activities. Perform other duties as assigned.
MINIMUM REQUIRED SKILLS, KNOWLEDGE AND ABILITIES:
Requires a Bachelor's degree in a social service field, behavioral sciences or related field.. It is
preferred that some experience has been specialized in the field of prevention services and/or
community networking. This position requires occasional interaction with Spanish speaking clients who have a limited understanding of the English language or who may speak only
Spanish.
